Improving Health in Sub-Saharan Africa: From Patient to Population – Data for Improved Health Outcomes in Cross-Border Settings

Executive Summary

Jembi aims to support the objective of achieving improved health outcomes utilizing disruptive healthcare technologies by working with the consortium on two main tasks. At the regional level we will equip and empower key stakeholders to make data-based healthcare delivery decisions, and at the patient/provider level we will help to improve health outcomes for all individuals by implementing a cloud-based personal electronic health record system to integrate cross-border data collection and ensure continuity of care regardless of geographic location, in the domain of immunization.

This work will be done in in collaboration with stakeholders, including Ministries of Health in the target regions, and other NGOs and implementing partners that are operating in the region. The strategy is leverage existing successful programs, and integrate existing sources of data into a broad analytics platform which will produce knowledge that will be use to inform decision and policy makers at a high-level, to improve delivery of services to populations, and to improve clinical care to individuals.
